Consent Form

1. Voluntary Participation

By receiving a tattoo, the client confirms that they are doing so voluntarily. The client also acknowledges that they are not under the influence of alcohol or drugs.

2. Potential Risks

The client understands the potential risks involved in tattooing, which include but are not limited to:

• Infection
• Allergic reactions to pigments or needles
• Fading or blurring over time
• Scarring
• Local swelling and redness

3. Responsibility

The studio will not be held responsible if aftercare instructions are not properly followed. The client is responsible for maintaining the tattoo after leaving the studio Premises.
